  • Citizenship Status

For example, a U.S. born citizen adult who wants to replace his social security card won't have the same required documents that a non-citizen child who needs an original one. A U.S. born citizen may use his or her's birth certificate or passport to prove citizenship and a driver's license or passport to provide identity. However, a non-citizen will require other documents.

Note : All documents submitted must be either original documents or a certified copy by the issuing agency. Photocopies or notarized copies of a document won’t be accepted.
